It's time to register for the 2009-2010 Homeschool Year!!!
Vocabulary:
- CSEF - Church School Enrollment Form (Fill three (3) out for each child. Do not put multiple children on one form.)
- RSR - Request for School Records (Optional - Fill one (1) out for each school that you need records from. Do not put multiple schools on one form. Please include $5 postage for the records to be forwarded to you. These are for your records only and completely optional.)
- FIF - Family Information Form (Only one needed per family.)
Enrollment Procedure:
- Please read, understand, and familiarize yourself with the Everest Academy Handbook.
- Print, fill out, and mail Everest Academy the following:
- Enrollment Check List
- Family Information Form
- Church School Enrollment Form (3 per child)
- Request for School Records Form (optional)
- Enrollment fee
- Self-addressed stamped envelope
- Make sure you sign and date all areas and mail to Everest Academy within two weeks of signature. Please pay special attention to the CSEF and sign both the top and bottom portions.
- Everest Academy will process all paperwork and return the RSR (optional), two (2) CSEF per child, and the Everest Academy acceptance letter to you.
- You will need to file one (1) copy of the CSEF per child and the Everest Academy acceptance letter in an easily accessible location.
- Forward the original CSEF per child to the appropriate city or county Board of Education Superintendent. See list below for addresses.
- Forward the RSR (optional) to the appropriate school.
Please remember that this is a busy time of year. You may expect your paperwork to be returned to you within about two weeks. If you have not received all paperwork within two weeks, please contact Everest Academy. Don't worry, you are considered enrolled when Everest receives your enrollment forms.

- Can I fax or email the paper work in? No, the BOE requires original signatures.
- Can I combine payments of fees? Yes, please make sure to fill out the Enrollment Check List.
- What form of payment is accepted? Cash, check, money order, credit/debit card (through Paypal), Paypal, or Cashier's Check.
- I homeschooled last year, do you need past school records? No.
Board of Education Superintendent Addresses
Once you receive the enrollment packet back in the mail, you MUST forward the original signed (by the parent and the administrator) Church School Enrollment Form to the local city or county Board of Education Superintendent of the student's public school district of residence. If you do not forward this form, then you are not legally homeschooling.
