Handy Homeschool Forms in PDF Format

Click on the Form Name to bring up a pdf file. Feel free to use these in your non-commercial homeschool setting.

Weekly Assignment Sheet - I fill out one of these for each child every week, usually on Friday after reviewing the prior week's work. Some subjects are only done on certain days, so I just highlight the appropriate days and pencil in the assignment (such as page #s). The small box to the right is used by the student to check off their work as they go.

Homeschool Rules - Print this out and put it in a document frame to hang on the wall.  It sets clear expectations, in a short list of rules.

Elementary Reading Log - I give one of these to each elementary age child every month.  I write their reading goal (usually number of pages) on the top.  At the end of the month, the kids get little rewards for meeting or exceeding their reading goals.

High School Reading Log - This provides a little more detail for high school students to track the books they've read.

Sample Weekly Schedule - Here is a sample school schedule for reference.

Sample High School Transcript - A great format for college and scholarship applications.  This format has been used successfully by both of my homeschool graduates.  You can create a formula column to calculate the GPA average, but leave off the calculations when you print it for submission.

Historial Fiction Report - This is a helpful format for constructing a brief report on a historical fiction novel.

Biography Report- Another helpful format for constructing a brief report on a biography.  It even includes a spot to paste or draw a picture of the person being reported on.
