My 5 year old daughter and I just finished her 2nd lapbook. This one the topic was All About Me.
We started out with 1 red folder which I did not trifold. We just used it as-is. (I don't know if that makes it a notebook instead of a lapbook, but to make my life easier I'm calling it a lapbook.) On the front she used stickers to write out All About Me, her name and decorated with a butterfly she traced, colored and cut out. Inside on the left hand flap I sewed 6 ziploc bags onto cardstock and we gluesticked the cardstock to the folder. Inside the flaps we used Make Your Own Book printouts from this site. These little book sheets fit perfectly in our ziplocs and they are designed to draw a picture at the top and write along the bottom. She wrote and illustrated the following:
I like to be nice
I have 2 brothers
This is my Mom and Dad
This is my dog Elvis
These are my cats Natomas and Pearl
My favorite food is turkey
I like Ballet
I like to go to Church
I like to go to the Theater

On the right hand side of the folder I sewed four 81/2 X 11 sheet protectors directly to the file folder. The sheets we used on this side I got from 3FoldChord. She posted about a lapbook she and her son made and was gracious enough to email me the file so we could use it too! For the "What Makes Me..." ones we took pictures of her making the appropriate expressions! (This was 3FoldChords great idea!) This was her favorite part! LOL

For Now and Then Information the sheet has weight, height, eye color, hair color, siblings, address, price of bread & milk, Preident, VP, and we added Price of gas. (Which shockingly has gone up $1.13 per gallon in her short 5 years!!!) At the bottom I made notes for her about being born at home and getting to go see Vice President Cheney speak the day before the last election.
For the family tree I just had her tell me all the names of our relatives. Grandma's, Aunts Uncles, Siblings, Cousins, etc. We just glued them on randomly. It would be nice to make an actually family tree chart if she was older.
In all the empty spots she traced around her hand and made little lift flaps. In these she put info like I am 5, I like Ballet & I like dogs.

The final thing we did was write one thing she liked for each letter in her name. For example if her name were Sam (Which it isn't) we would have done the following:
S oup
A apples and Ants (She wrote that one becuase she has an A!)
M oney
So that is what we did in a nut shell! (Or should I say in a file folder! LOL)
