Help me diagnose a rash!
My son has a rash, and after a ton of
research on-line I am baffled. At one time I thought it was
Roseola, but there was no fever. I thought it was Fifths Disease,
but the rash did not start on the face, but on the torso and it is not
lacy, but splotchy. I even considered Scarlett Fever, but again
no fever. And I have finally given consideration to an allergic
reaction to Amoxicillin--which he just started a week ago, but still
not sure.
Daniel started his Amoxicillin last Saturday for a
strep infection on his bottom. He was otherwise very healthy and
happy. Out-of-town family came to visit on Monday and had a sick
child (runny nose, fever). Daniel was exposed to whatever they
were carrying. I wasn't too concerned, however, because he was on
antibiotics.
On Wednesday, during bath, I noticed some rough
skin on Daniel's sides and stomach. It felt like excema or sand
paper (thus, the Scarlett Fever consideration). But it wasn't red
nor were there any bumps or spots.
Around that time Daniel
also developed a slight cough and runny nose. As of today,
Sunday, he has a pretty rough cough (not consistent or constant,
though) and a runny nose.
The rash began to develop over the
next few days. The red spots/bumps began to appear on his tummy
and back and sides. By the next day they were covering most of
his torso and legs and a few on his face. By yesterday and today
he was completely covered with the spots--even on the palms of his
hands and the soles of his feet. The spots and redness on his
face became much more apparent.
Except for the cough, a loss
in appetite and a slight bit of irritability, Daniel is fine and
healthy and happy. He still plays and laughs and runs
around. He wants to be held some, but not more than usual.
The rash does not seem to irritate him and it does not appear to be
itchy. I don't think he even notices he has a rash.
Finally, I'll add here a few pictures.

Are there any ideas? I'm not concerned yet because he seems just
fine and he has no fever (and has not had a fever). He did vomit
for no apparent reason what-so-ever on Tuesday before the rash
appeared. Maybe it is more related than I had originally
thought. Today he seems to be at the height of the rash. It
is at its worse right now it seems.
We will contact our doctor
tomorrow (since today is Christmas) and ask about the consideration of
being allergic to Amoxicillin. We have stopped giving it to him
completely as of right now.
And a related question: how did he
get sick (with the cough and runny nose from his cousin) if he was
taking antibiotics?! That one gets me, too!
So any thoughts or comments?!
Thanks!

My 13 yo is allergic to amoxicillin. Your son's rash looks exectly like his did when he took amoxil as a 1yo, and it started the same way. The doctor we went to at the time said he could go into anaphalactic shock if he kept taking it. Personally, I would stop giving it to him until you can see the doctor, just in case. I'm not the least bit surprised he picked up a cold while on antibiotics, especially if he's allergic to them ... his immune system is probably low right now, fighting the allergy. Of course I'm no PHD or anything!

I will also vote for an allergic reaction. I am allergic to Eurythomyacin (sp?) That rash looks exactly like the one that I got oh so many years ago when on the that drug -- though no one in the medical community believes me know when I say am allergic to it. . . . . .
A cold is a virus - which is not touched by an antiobotic.

Laura, my youngest had a similar rash about a year ago. We never really determined the cause of it as the pediatrician said it was probably viral. It eventually went away....I know the ped. gave us a prescription, but I don't remember what it was....
So glad he is feeling better now!

While scouring the 'net for info on the Draw Write Now series, I found your blog. This is my first time responding to a blog. ;)
I know that your son's rash is almost 12 months old, and unless he has another round of the same a'biotics, you won't know for sure if it is the a'biotics that caused it.
Children get many and varied rashes that come and go, however I am thinking that your son may well have had, German Measles - Rubella. Several of mine have had it,and his symptoms sound much like my kids were.
All the best

I;m researching rashes, having just discovered one on my 12 month old. His looks just like the photo of your son, with pink/ red sandpaper like bumps on his belly and chest - so now I'm wondering - what did your son have???
I'm thinking/ hoping it's just roseola, but so many photos present different views. Yours is the first one that looks just like it...
Thanks,

Hi
My son has that exact same rash - however i can't think of anything he could be allergic too as nothing has changed in his eating/contact etc. It has been suggested it is German Measles. How long did the rash last on your child?

I just came from the Dr. my son had the same exact rash following the same symptoms. He was on Amoxicilin and the rash developed a week later, he even got a cold as well. The Dr. says it is an allergic reaction to the antibiotics he says you can tell as they are on the palms and bottom of feet as well as not all spots are symmetrical and have "targets"in the center. they range from splotches to dots. He suggested I give him Benadryl and wait for it to subside. Hope this helps.

I could just write "ditto" as so many of the other mother's have described the exact rash my son now has. He too has just been on Amoxicillin for precisely SEVEN days! His rash gets worse as he heats up from exercise. Have any of your children's gotten worse with heat? The doctor says Benadyrl for now and bring him in the morning to check for a strep rash.

I have a six month old son that has the exact rash he started it yesterday. I took him to his Ped and she said it was from the amoxil he had been taking for the past 8 days. Today the rash seems to be worse because Zach got hot in the car. I have another appointment w/ the Dr. in an hour just to double check. Your son's rash looks just like the one on my baby. He is showing no real discomfort but his appetite is down.

This update is LONG overdue, but I had not realized how many people have been to my post about my son's rash that happened over a year-and-a-half ago! It is amazing how the internet works. I'm hoping that what people read here may be helpful and beneficial to them.
There's no shocking ending to this story. I took my son to see his pediatrician the next day and she said that it looked like a viral infection. He was also suffering from an ear infection and a cold. She gave me an Rx for another type of antibiotic and something for the sniffles. She gave me nothing for the rash. She felt pretty sure that it was not an allergic reaction to Amoxicillan, though, but I can't remember why now.
I also remember that the rash peeked the day I posted and that it dissipated within the next day or so. I guess it lasted a total of 3 or 4 days at varying degrees.
My family has not encountered any other such rash since then! Daniel has not been on Amoxicillan either since that time, so whether or not we can debunk the allergic reaction theory has yet to be determined. Although, I'll go along with the ped. unless I have have something to conflict with her diagnosis.
Hope this has helped someone. It is so frustrating when you don't know what is going on with your children. In all the cases of my children, it's been reassuring to come on the internet and learn as much as I can about their illnesses and problems!

I was looking for pics of rashes to identify my 16 mo's rash and saw this. It looks just like a rash my 3 yo had when he was about 10 mo. He had just been put on Amoxicillin for his pneumonia. The big difference with his reaction to others posted here is that the rash showed up just hours after taking the first (and only) dose of the Amox. Unfortunately we had just run out of his "usual" soap and had used something new we'd just bought for his bath that night. It was the only "baby" soap we had in the house and we didn't really think about him being allergic. The rash showed up just after the bath. We called his ped and she said it was probably an allergic reaction to either the Amox or the new soap. We still don't know which it was, but we always list penicillin (Amox is a penicillin) as something he's allergic to now. He does have some sensitivities to some laundry detergets/dryer sheets and gets an itchy rash on his neck if we accidentally use them on his clothes, so I still wonder if the reaction was to the new soap.

My 14 mo dd has just taken her first dose of amoxicillan and, after reading the drug information sheet I, too, am on the lookout for all side effects. I thought it may be a help to parents reading this to note that a rash is a common side effect to amox and does not always indicate an allergy. However, it is hard to distinguish between an allergic and non-allergic rash, so its best to check with a doctor to be sure. (Pretty sure my dd is fine; about 20 min after this first dose her skin was very splotchy but likely due to fever -- will continue to watch her, which is one of my favourite pastimes anyway.)

I am a very hard time trying to figure out why my son has the same rash as your son did. However, his rash has lasted almost two weeks now. I took him to the immdiate care center. They tested him for strep throat it came back negative. I can't think of any changes in his food soaps. I ask the doctor about a bold test. His extact words was "That type of test does not exsist". I was not hapy with his answer. He advised me to continue to use the AVENO products and it should go away on it's own. I am researching about different rashes and I am getting scared b/c I don't know what it is. The rash really does not itch, no fever, no nothing. Just the rash.

My son has the exact same rash and it progressed quickly over a period of minutes. It started on his back and stomach then progressed up to his scalp. He was diagnosed with scarlet fever. You really should have him checked since scarlet fever when left untreated can be fatal. Usual treatment is antibiotics for 10 days.

Ok, I know I'm like 3 years late here and your kid is probably doing algebra now, but I wanted to say thanks for keeping the picture up because it helped me diagnose my daughter. The rash is a side-effect of amoxicillin, not an allergy, unless it itches or turns into hives. 5-10% of kids who get amoxicillin will get that rash, and it's harmless... unless it itches. Kids who get the rash can even be given amoxicillin again, and rarely develop the same rash.

I just came home from work, I work in a pharmacy as a technician, and went to tuck in my daughter. she was in bed scratching from head to toe.. she has a rash that practically covers every single inch of her body.. it is red spotty and slightly raised... her face has a mask across her cheecks and nose that is red splotchy with white (sort of looks like a brand new misquito bite), mine get white and swell with ink around the edges... I called our after hours clinic and the advice nurse called me back.. after ruling all possible irritants out we were still baffled... she was at the doctor exactly six days ago and they put her on amoxicillin for a sinus infection.. her rash itches really bad.. I don't think your son had that issue!! they said that the amoxicillin rash is quite common, but that it rarely ever accompanies itching, also an allergic reaction is less likely as time passes on the medication.. she had scarlet fever when she was around two. i had no idea she was sick, she presented no symptoms until I noticed the rash. My daughters rash now looks similar to that.. it is red splotchy little spots over entire body (more noticilbe in crevases), it usually occurs within 6 days of the onset of strep and within ten days the skin starts to peel and the old skin is leathered and sluffed off.. the nurse I spoke with this evening suggested it may be a viral rash.. Honestly until then I didn't think kids could get a rash from a viral infection, but after finding your post, and reading other responses It seems all to possible.. will see how my daughter does in the am.. and I'm glad that all is well... my daughter is nearly 9 now!

Hello,
I am so glad I found this website. My son started taking amoxicillin last Monday night. No problems whatsoever. Until today that is. He woke up from the nap with the same rash on his face, back, legs, groin, arms-everywhere! so, I thought it was a heat rash. He did have a fever this AM so that is why I thought this. Anyways, I am not giving it to him anymore and I will call his ped tommorow and I hope he doesnt try and tell us that its a viral infection. He gave it to him because of his ear infection already. I am praying it goes away fast because he has had one ailment after another ever since he started daycare nearly 3 months ago...good thing I am not working tommorow. Thanks again.

I took my son in for strep last week. Amoxicillin was perscribed. A week later, the red rash. I thought it was chicken pox for the last couple days, until I saw the pics of your child. I was actually searching to see if he could go back to school, because this has happened over the weekend. The curious thing is the ears and the cheeks. They seem to be more splotchy if you will. The red dots are all over his torso and arms and face but the ears and face were more puffy. This made me curious because there were none in his hair or between his legs. Now I realize after seeing your posting that we finished giving him the amoxicillin Friday. This is Sunday night and the splotches are close to gone. Though he has had amoxicillin before without having any allergic reaction. I'm fairly certain from these similar conditions he is now. Thank you for your posting.

The only difference between this in adults and children is that adults usually get swollen joints almost like arthritis. I has exactly that and it was horrible, I couldnt even walk!

Mar. 14, 2008 - Amoxicillin rash often misdiagnosed as being allergic
Posted by JenRN
My daughter was on Amoxicillin 250mg twice a day for a week then developed the exact same rash your son had in the photo. It first started as a couple of red dots on her trunk then a few hours later spread all over her abdomen, back, thighs, and arms. I thought it might be Roseola but turns out her Pediatrician said it was an "Amoxcil Rash". She said that there are two types of Amoxil rash. 1. Morbilliform? 2. Urticaria. The urticaria is the hive type of rash that is rather large in appearance and itchy. this is apparently the true "i'm allergic to Amoxicillin" rash. The morbilliform rash is a common reaction that some kids get when they are on Amoxicillin treatment. Usually it is non-itchy. She also said that MANY people are misdiagnosed as having Amoxicillin allergy when in fact they are not allergic at all. Hope this helps. Our peds doc is pretty sharp and after reading all of the posts i know she's right again this time.

Can I just say thank you my son who had been on amoxcillin (as he has large tonsils and adenoids - waiting for a date for them to be removed!!) and finished the course a few days ago has come out in the exact same rash! We took him to the doctor who seemed to think he had chicken pox the spots are everywhere feet, hands, hair, tummy, legs, and arms but they seem to irritate more on the ears and face/ cheeks, that was yesterday but on going back again today as I was sure it wasn't chicken pox I was told he's allergic to the amoxcillin (he's also allergic to eurmycin? probably not spelt right!) and that he has the strep rash we're now on good old penicillin 4 x 5ml per day he's taken 2 doses already and his cheeks are looking less angry.
So MANY thanks for this having read all the other replies I'm not the only one, really useful to read everyone's comments!

My son has a very similar rash as first described. He has not been sick and is not on any meds. It started while we were in AZ and we are now back in MI. It started as dry scaly bumps on the sides of his stomach. Yesterday it spread all over the trunk of his body and bum. He was scratching his neck a lot. Eating, playing, drinking normal. The Dr said it was probably a delayed reaction to sunblock or chlorine but I doubt it. His skin is so dry and it feels like sand paper bumps. Bizarre. It got a little better today, its not as red but its spreading down the back of his legs and its on his face. I'm perplexed as to what caused this.

My 18 month old daughter woke up one morning on say 8 of taking amox. with the EXACT SAME RASH. I took her to a pediatrician in our small town and he said, "I am not sure it is the amox. it is too late by now and it probably viral." That did not seem right after lots of research. I then took her to a Children;s Hospital in the captial of our state. A prestigious hospital for children and ALL of the doctors confirmed it was the Amoxicillin. It CAN take days and days for the allergy rash to arrive. Her rash got worse with heat and sun. It was EXACTLY THE SAME as your sons.
Please dont let them give your son anything with pennicilin. He could go onto something worse.
I saw specialists and they confirmed it.
I hope this helps you.

My daughter (now 3) got put on amox when she was 11 months old. She broke out from head to toe with a rash and was pretty irratable. The doctor didn't perscribe anything for her rash when we went back. I got some cortizone10 and applied it to help with itchiness (although i didn't know if she was itcy) the rash went away after 4 days. and i thought that it was over. That same day my daughter broke out with a case of thrush.. this was the first time i had ever even heard of that and it was pretty scarry.. the thrush was gone after only 2 days. that evening she began to get a high fever. She ended up having to go into the hospital because the infection spread causing her to get a kidney infection. 6 days she spent in the hospital and her doctor swears up and down that it wasn't caused by the amox. i beg to differ..... My 13 month old son just got perscribed amox for the first time. he took it for 2 days and yesterday broke out from head to toe with the same rash that my daughter had when she was younger. Of course that was the first thing that popped into my head as it was difficult for me to watch my daughter go through that. I immediately called the doctor and he advised me to take my son off the amox immediately.. also told me to give him some benadryl to counter act the allergy... unfortunatly they don't make benadryl for infants.... i had to use childrens benadryl and now have to watch my son like a hawk to make sure he's handeling it well.... The rash still hasn't gone away and this is the 2nd day taking the benadryl.... i really don't want to see another baby spend 6 days in the hospital... that would totally suck!

My 23 month old has had a rash for the past few days. Prior to the rash he was in a horrible mood, didn't want to eat, and only wanted to lay around. I took him to the ER as I was worried about dehydration, being that it hit 42C here the day he started acting strange. Doctor said he has a viral infection, which didn't quite wash with me. But either way, the rash you have pictured is identical to my sons, it doesn't appear to bother him whatsoever, and seems to get a little worse during play time, or bath time, which I assume is from the heat raising his body temp, and therefore aggrivating the infection.
Nonetheless, thank you, your blog has put my mind at ease!

My daughter is 12 ys old now, but the first time she took Amoxicillin .. around 12 mos??.. she got the red rash exactly where the medicine would have traveled from her mouth to her belly. I never gave it her again and she has never had a rash like that again. Funny though that Amoxicillin is in the penecillin family, but she can take other penecillins just not Amoxicillin. However, I prefer to give her Zithromyacin ??(Zithromax) and it works great.

Our 16 month old has almost the exact same rash, and was also was an amoxicillin for 7-8 days before the rash suddenly appeared. (My wife also noticed it for the first time when he was in the bath and the rash first appeared on his stomach and back and then spread to his entire body with the same rosy cheeks).
From the limited research I have done on-line , it is hard to know for certain if this rash definitely indicates whether he is allergic to penicillin or not (although the doc said he is);
However, I found a website that discusses the allergy and a skin test that can confirm either way.

After reading all the parents comments..I really think it's got to be from the Amoxicillin!! Whats with these docs? And give me something more than a "viral rash"!
I was told to continue with the medicine...just 1 1/2 more days.
Thanks for your post.

Mar. 26, 2009 - Allergy to Amox VS. Scarlet Fever
Posted by Renee Mena
I seen that there was a comment saying this rash was caused from scarlet fever. I am also familiar with this illness with my daughter. (a separate incident) It can only be scarlet fever if the strep virus is present. You can be tested for this easily by having your dr. swab your throat.

Apr. 28, 2009 - Just go to the doctor.

Rashes are hard enough to diagnose, and even diseases that tend to come with certain rashes can sometimes have "atypical presentations" as the doctors say. (For example, I know someone who instead of getting the bullseye rash with lyme disease, got a rash that looked like hives.)
My 3 year old daughter has a rash that looks VERY similiar (except that her cheeks don't look quite as bad). She also has eczema, so I'm a bit used to her having weird dry rashes. When she came down with the rash a few weeks ago, which started on the palms of her hands(!), I called the Dr. and together we decided to take the "watchful waiting" method. She had a slight fever but other than that seemed to feel fine, she said she didn't have a sore throat, the rash only itched at night (not usual for her), and I guess cold viruses can do weird things in people with eczema. At this point the rash was only on the palms of her hands and soles of her feet and blanched when pressed on. Two weeks later, she still has a little of the rash left on her hands and feet, except for now when she gets up in the morning she's covered in little dots/pimples that fade as the day goes on. I brought her into the Dr. who said that it was -probably- a virus but that they'd run a strep test just to make sure. Strep came back postitive... confirmed diagnosis of scarlet fever. No biggie, 10 days of AMOX and she'll be good as new, and even without treatment it would have probably gone away on it's own. Instances when the strep BACTERIA cause a serious reaction are very rare, and heck, even the flu can be fatal in some rare cases. ;)
HOWEVER, what is interesting to me is that like several commentor's kids she was treated for a double ear infection less than a month ago, except for that she got Zirtomax and not AMOX. I asked the doctor if the strep had anything to do with the ear infections and she said no, but after reading it repeatedly on the internet and seeing all these comments, I'm wondering if it does. ;)
Anyway, that's just my experience for all the worried moms out there. ;)

Jun. 26, 2009 - same rash

Just a word to those of you who read this and felt this type of rash was related to the antibiotoc. Our experience was different. My 7 year old had an identical rash along with a fever of @ 101, 102 before we figured out that he tested for strep (never once complaining about his throat!) and was then put on an antibiotic. His older brother never got much of a fever but has just now broken out in a rash also. All this was before the antibiotic.
